To be included, a death benefit had to be claimed. If a claim was not
filed, a name isn't necessarily included in the Index--that's another
reason folks don't always show up. My mother died in 1984 at the age of
62. No benefit claim was filed; she's not on the Index. My sister,
grandmother, father and brother-in-law died in 1996, 1996, 1998 and
1998, respectively. Claims were filed for each and each is included in
the Index.
