Debby,
I have received many SS-5 copies from the freedom of information officer. I
have not had the same problem as you, but have had others, such as when a
lump sum payment is not made. A monthly payment to survivor causes the
individual to not be in death index. Also, my grandmother had a SS# but did
not work. After her husband passed away, she was forced to use his SS# as
her own. Luckily, I found grandpa's SS#s in the death certificates.
