Hi Mary,
on birth certificates maiden names will be used therefore there will be no
'né'! So one of your 'names' will be the mother's - no matter how 'male' it
looks!:-)
The quoted text: "Aláirás elötti esetleges megjegyzések" means 'notes prior
to signatures'. And "Aláirások" means 'signatures'. So if any of the signatories
wanted to add something to the document that should show up over the signature
(of the person).
