Since at least from 1989 (year of my mother's death), the death benefit has
been payable ONLY to a surviving spouse (with the exception of some
qualifying dependants, usually minors already receiving benefits, as would
be the case with a deceased father). Generally, however, if the spouse
doesn't survive, then no death benefit. Prior to 1989 the benefit went to
any next of kin. I hadn't heard that it went to the funeral home if their
were no survivors, but that could be true. Note that "column B" - "last
benefit" address - does not necessary mean "death benefit" - it could be the
last regular benefit, for example.
